Worthy State Officers, Directors, Diocesan Deputies, District Deputies, Committeemen,

There are still 63 active councils in Texas which have yet to submit their Survey of Fraternal Activity Form 1728. This form was due on January 31. This form is vital to Supreme being able to defend ourselves as a charity.

Here is the list of these councils.

Congrats to Fort Worth and Tyler for having all their forms submitted.

If you are a Diocesan or District Deputy with councils on this list, PLEASE do everything within your power to get this form submitted ASAP. Don't just forward this e-mail! Carry the form to their meeting and get them to fill it out and submit it.
